Application Order

Please place your completed application in the following order:

• Self photo (3x5)
• Completed Application Form and all applicable forms in the MLK and AW packages
• Sealed high school transcripts
• Letter of acceptance/continued enrollment to Institution of Higher Learning
• Official proof of household income and number in household
• Typed essay (225-250 words)
• 3 Letters of recommendation
• Letter of hardship, if applicable
